Please sign and submit this directive before you get sick or injured in order to insure you receive no medications, surgeries, treatments or disease therapies that have been tested or tried in research animals.
 
An ADVANCE ANIMAL DIRECTIVE conveys to your doctors, paramedics, pharmacists and other health care providers the kind of health care you would like to have if you are ever rendered unable to make medical decisions (if you are unconscious, for example). If you are admitted to a hospital, the hospital staff will likely speak with you about your advance animal directive. Advance animal directives can take on many forms as laws are different in each state. You should be aware of the laws in your state prior to signing this directive.
